The Healthy You Program at Children’s Hospital of The King’s Daughters
The Healthy You Online Chat Room allows the children that are enrolled or have completed the program to attend scheduled online follow-up discussions about weight management with the licensed clinical social worker and their peers. The Healthy You Program focuses on ages 8 to 18 years and in order to comply with The Children’s Online Privacy Protection Act of 1998 and other regulatory requirements, CHKDHS must obtain parental consent before a child is allowed to participate in the online discussion and CHKDHS must disclose its practices for the collection, use, and/or disclosure of your child’s personal information.
Parental consent is required in order to permit CHKDHS to collect, use, or disclose your child’s personal information and for your child to participate in the chat room for the Healthy You Program. By completing the consent form and authorization form provided to you by CHKDHS, you agree to your child’s participation in the chat room and permit collection, use and disclosure of your child’s personal information in accordance with the “Collection of Your Child’s Information” and “Use and/or Disclosure of Your Child’s Information” sections below. Each time that a user signs onto the chat room, the user must click the “I agree” button (at the bottom of this page if viewed online) to acknowledge receipt of and agreement to the then current CHKDHS Online Chat Room Terms of Use and Notice of Information Practices with Regard to Children.
If at any time you wish to end your child’s use of the CHKDHS Online Chat Room, you may click on the “I disagree” button at the bottom of this Web page - if you are viewing this notice online – and enter your child’s username and password, or notify the Healthy You Program (Healthy You Program, Children’s Hospital of The King’s Daughters, 601 Children’s Lane, Norfolk, VA 23507) in writing that you wish to terminate your child’s participation in the chat room. Upon receipt of a request to terminate access, your child’s account will be deleted and no longer maintained in retrievable form along with all personal information previously collected by CHKDHS. CHKDHS, in its sole discretion, may also terminate any user’s access to the chat room for inappropriate use of the chat room.
CHKDHS recommends that you supervise your child while he/she is participating in the CHKDHS Online Chat Room as your child may post personal information that will be available to other users logged into the chat room.
CHKDHS values the safety of your child’s personal information and wishes to collect personal information from your child to enable him/her to participate in the Healthy You Program. CHKDHS is prohibited from conditioning your child’s participation in an activity on the child’s disclosing more personal information than is reasonably necessary to participate in such activity. CHKDHS intends to directly collect the minimum amount of information required in order for your child to participate in the CHKDHS Online Chat Room; such minimum information includes your child’s username and password. Your child’s username should not include any information that identifies or suggests his/her age, name, gender, current location, the location where he/she lives, or the school that he/she attends. Your child’s username will be used only to identify the messages that he/she enters into the chat room. Your child’s password is collected to allow access to the chat room and will be secured and encrypted.
Once logged into the chat room for the Healthy You Program, your child may make additional personal information (individually identifiable information) available through the chat room; such postings will be passively collected by CHKDHS and will not be censored or modified in any way by CHKDHS before they are made public to other users who have logged into the chat room, unless they contain words or phrases that are deemed inappropriate by CHKDHS. Therefore, CHKDHS recommends that you supervise your child while he/she is participating in the chat room.
CHKDHS’s use and disclosure of your child’s personal information is limited to the operation of the Online Chat Room and to the extent required to comply with or permitted under state and federal laws. Any personal information that your child posts in the chat room will be available and disclosed to other users who have logged into the chat room. CHKDHS does not control or limit any further disclosures of any personal information posted by your child in the chat room. Once personal information is made available publicly by your child by posting it in the chat room, CHKDHS may also use and disclose his/her personal information for general internal recordkeeping purposes as set forth below. Because CHKDHS does not accept advertising on its website and does not allow commercial access to visitor information on its site by third parties, your child’s personal information will not be disclosed to third parties except as set forth and permitted herein. Your child’s information will be retained in our system until he/she completes the program and follow-up, or until parental consent is revoked, at which time your child’s information will be deleted and no longer maintained in retrievable form.
As part of general internal recordkeeping, a log is maintained for all discussions in the chat room. If CHKDHS deems it necessary, CHKDHS will contact the child’s parent or guardian to discuss any issues involving your child. CHKDHS may provide copies of or give you an opportunity to review the text or personal information provided by your child that is relevant to your child upon request.
