Referral, Admissions and Fees
All weight management clients must have the approval of their pediatrician or family physician.

Fee for Service
The cost for the ten-week Healthy You lifestyle class series is $350.00. This includes your ten week lifestyle classes, eight week YMCA pass and access to the CHKD Healthy You on-line discussion board for continued support and interaction. Payment must be made prior to the first class or appointment. All fees are non-refundable. Scholarships and fee adjustments are available based on ability to pay. The Clinic Assessment with the Healthy You program team will be billed to your insurance company. You are responsible for the co-pay.
Psychotherapy group participation as well as family and/or individual therapy sessions are an additional charge and may be covered by insurance.
Checks should be made payable to CHKD and mailed with the registration application and a copy of your insurance card to:
