Patient Services

We have several online services to assist you. Through the CHKD website, you can pre-register at our practice, schedule your next well-child appointment, request your medical record and refill your prescriptions.

The process is simple. First-time users will need to create an account. After your account registration, login to our site to complete the online form of your choice.

Note: Please add webteam@chkd.org to your contact list so you will receive all confirmation emails. This prevents the messages from going to your spam folder.

We accept many other insurance carriers. Please call our office prior to receiving services to see if our practice and/or your child's doctor accept your insurance plan. Our participation with the health plans and/or companies listed above is subject to change.
