Referral Guidelines

Our team provides acute and chronic pain management, as well as complex symptom management and supportive care for patients from birth to 21 years of age. We accept new patients under 18 years of age.

Please review our referral guidelines below to help ensure that your child/patient receives the best possible care.

How can I refer my patient to CHKD’s pain and palliative care practice?
  • Urgent outpatient consultations (providers) call Doctors Direct at (757) 668-9999 and ask to speak to the pain and palliative care provider on call. 
  • Non-urgent referrals (providers) call (757) 668-8616
  • Faxes (providers) can be sent to (757) 668-5676
  • Scheduling (families) can be made by calling (757) 668-8616
When will my patient be able to obtain an appointment?

Your patient will be seen at the earliest appointment available. Urgent referrals will be prioritized. If your patient’s insurance carrier requires prior authorization for specialist appointments, this must be completed before we can move forward with scheduling. If additional records or information are needed prior to scheduling an appointment, someone from our team will reach out to you. One of our staff members will call the patient to schedule an appointment.

What work-up is needed prior to the first appointment?

After referring your patient, please send any relevant office notes and documentation that are not accessible through CHKD’s Electronic Health Record (EHR). This information can be faxed to (757) 668-5676.

Where will my patient be seen?

The Pain and Palliative Care outpatient clinic is located at our main hospital on the second floor.
