CHKD understands that you are a unique individual. So are your wishes for the care you want to receive when you cannot speak for yourself.
Our Chaplaincy Services help patients and community members 18 and older understand their options and complete an advance directive, a legal document stating your wishes about how you would like to be cared for should you become incapacitated and unable to speak for yourself.
Chaplains assist you in preparing to put your wishes on paper so that your family and doctors will know the treatments you would like if you become too sick to tell them. You can schedule a time to meet one-on-one with one of our CHKD chaplains to discuss and prepare your advance directive.
In 1990, Congress passed the Patient Self-Determination Act. It requires health care institutions to tell patients and the people in their communities about their rights under Virginia law to make decisions about their health care. These rights include the right to accept or refuse care and the right to make advance directives about their care.
This booklet explains your rights under Virginia laws in effect July 1, 2009.
