Please call your physician and discuss any concerns you may have regarding a referral for occupational, physical, or speech therapy. If the physician deems a therapy evaluation appropriate, a prescription will be required to begin the scheduling process. Please fax the prescription to the CHKD Therapy Services Department at (757) 668-7389.
Wait times vary at each health center location. An office staff member will contact you upon receiving the prescription for therapy services, in order to discuss your location preferences and times.
Yes, please come 15-20 minutes early. This will facilitate a timely visit with your therapist. You may also complete the case history form or the literacy case history form to expedite your first visit.
Most insurance plans have a website you can access. You can also call the customer service number located on the back of your ID card.
Please call our office if you are going to be late or cannot keep your appointment. The office staff can assist you with rescheduling if necessary. We do have a $25 charge for missed appointments if they are not cancelled 24 hours prior to the visit.
No. You may choose the location you prefer your child to receive therapy.
Yes. Our electronic medical record allows us to coordinate care easily among all of our locations, as well as among providers.
Yes. When you leave a message at any of the offices, your call will be returned either by an office staff member or one of our therapists. As always, if your child has a life-threatening emergency, please call 911.
