Referral Guidelines
Our team provides evaluation, diagnosis, management and treatment of allergic and immunologic disorders for patients from birth through 21 years old. Adult appointments are available for specialized diseases, such as hereditary angioedema (HAE), but must be authorized by a physician.
Please review our referral guidelines below to help ensure that your child/patient receives the best possible care.
- Urgent consultations (providers) - externally call doctor’s direct at (757) 668-9999 or internally message one of the physicians directly in message center.
- Non-urgent referrals (providers) - internally should utilize the Referral Management System, and external referrals may send a fax paper referral to (757) 668-9444.
- Scheduling (families) call (757) 668-8255
Note: Patient input for appointment scheduling is required in order to schedule an optimal appointment time. However, referring providers will be notified if a patient referred via phone is unable to make an appointment or if they are a no-show. All scheduled appointments can be verified within Epic.
There are no immediate appointments available. If your patient's referral is urgent, please do not hesitate to contact the office directly to have them scheduled.
Please provide us with the reason for referral, a list of current medications, and medication delivery devices. We also need any relevant office notes, labs, previous skin testing results, immunization records, and any relevant chest and/or sinus x-rays that are not accessible through CHKD's Electronic Health Record (EHR).
Note: Patients should stop oral and intranasal antihistamines seven (7) days before their first visit, if possible, and should allow 2-3 hours during their first visit for any necessary testing.
Allergy and immunology offices are located at Fort Norfolk Plaza (Norfolk), Harbour View (Suffolk), Oakbrooke (Chesapeake), Oyster Point (Newport News), and Landstown (Virginia Beach).
