How can I refer my patient to CHKD's gastroenterology motility program?

For any facilities referring from outside of CHKD, please complete this CHKD GI Motility referral form. Fax the completed referral form, along with any relevant office notes, prior testing (ex: Gastric Emptying Scan, Abdominal X-ray, CT scans, Sitz mark studies, Video Swallow Studies, UGI, Barium Swallows, and/or previous EGD/Colonoscopy Biopsy results), labs, growth charts, and a copy (front and back) of the patient's insurance card to (757) 668-8274, Attn: GI Motility Program.

If you call to schedule, you will still need to fax the referral form and any pertinent records. For a more thorough visit, we will schedule your patient’s appointment after we have received the completed referral form with the requested notes.

All motility testing takes place at the main hospital and some testing requires an inpatient admission.

Initial outpatient appointments and follow-ups are offered at the following locations: CHKD (Norfolk), Harbour View (Suffolk), Oakbrooke (Chesapeake), Oyster Point (Newport News), and Concert Drive (Virginia Beach). With the current restrictions due to COVID-19, a telehealth first visit may be appropriate for patients needing to travel, depending on their insurance.
