Please send any relevant materials not accessible through CHKD's Electronic Health Records (EHR) such as lab results, radiographs, and other accompanying reports. Some conditions require pre-consult work-up. Please contact our office at (757) 668-6550.

No pre-referral work-up is required for most conditions. No advanced imaging studies (i.e. CT or MRI) are needed before referral. Please fax pertinent medical records to (757) 668-6544.
